A SOAP assessment is a structured method used in healthcare to document patient encounters and progress. It stands for Subjective, Objective, Assessment, and Plan, representing the four key components of the assessment: the patient’s reported symptoms and history (Subjective), objective findings from examination or tests (Objective), the healthcare provider’s professional judgment and diagnosis (Assessment), and the proposed treatment or management plan (Plan).
